How to make a referral

Alternatively, our support team can post a referral form to you to complete. Simply send an email requesting the form, along with your home address to contact@ypwd.info.

FAQ & Referral Criteria

Referrals can be made directly by you or by somebody else on your behalf. All referrals are carefully reviewed and assessed by our Area Managers.

If the person has not yet received a formal diagnosis, please refer them to their GP where the assessment can be started.

The charity’s support services are tailored to people who have received a diagnosis of dementia under the age of 65.

If the person was diagnosed over the age of 65, please refer them to the dementia services offered by your local health organisations. If you live in Berkshire this would be the Berkshire Healthcare NHS Foundation Trust. If you live in Surrey this would be the Surrey and Borders Partnership NHS Foundation Trust.

Younger People with Dementia currently provide support in Berkshire, Surrey Heath, North East Hampshire, Farnham and Woking.

Our charity hopes that with further funding we will be able to provide services nationwide.

To find services where you live, you can use Dementia UK’s searchable database of young onset dementia groups and services across the UK.

What happens next?

Once our team receive your referral form, we will arrange a face-to-face initial assessment with you. Our assessment will enable us to establish whether YPWD offers appropriate support for the person referred. 

If the assessment concludes that our services would be beneficial then we will arrange an offer of support. This may include some trial workshops and events to see what works best before committing to a full plan. If our services are not suitable for the individual, we will support in signposting them to a more appropriate service.
